Registering For Online Affiliate
Step 1: Provider Group Registration
The first step in gaining access to Kaiser Permanente’s Online-Affiliate tool for your authorized Practice, Group, or Facility is the Completion of our Provider Entity Agreement* by an Owner, CEO, CFO, VP or other Executive. This agreement outlines the conditions and responsibilities that the owner must assume for any individual user who associates themselves with the Practice, Group or Facility.
Unsure whether we have an agreement on file for your provider group?
You may verify by sending your group name and Tax ID to your regional representative at the email address below. Someone will respond to confirm if we have an agreement on file.

Step 2: New User Registration
Once you have received a confirmation email from KP-HI-OnlineAffiliate@kp.org for your Provider Entity Agreement, you may proceed with the with the New User Registration Process*
Note: Receipt of Signed Provider Entity Agreement does not guarantee requests for Online-Affiliate access will be granted
